The Enhanced Medicaid Transportation Program is designed to assist
individuals to Medical appointments.
Who
is eligible?

You
have a current Medicaid card for the same month as your
appointment.

You
have met your spend-down for the month.
-
Your
appointment is for a service paid by Medicaid, such as
counseling, doctor visits, prenatal appointments.

You
do not require ambulance or ambulate services or you are able
to ride in a car or van.

You
are pregnant and have applied for Medicaid.

Community
Connections Program
may be able to offer limited gas vouchers for school or
work.
Transportation
Options
Fairfield
County Job and Family Services uses three options based on
customer need and driver availability.

Lancaster
Public Transit is available for eligible residents who have
verified Medicaid reimbursable appointments.

Gas
Vouchers: Vouchers are issued to eligible participants who can
use their car or friend or relative's car for the
transportation to the Medicaid reimbursable appointment.
Vouchers can be redeemed at local gas stations in Fairfield
County.

Direct
Transportation: An employee of Fairfield County Job and Family
Services will transport eligible individuals to their verified
medical appointment. This transportation is typically used for
outside Lancaster or Fairfield County.

to Access Transportation Services

1-800-450-8845. An application is completed over the phone. To
ensure adequate time to schedule drivers or mail the gas vouchers
or transit tickets, please contact us 10 days prior to the
scheduled appointment.
