How can I
apply for child care assistance?
Complete an application and mail, fax to 740-687-6716 or
bring to Fairfield County Job and Family Services at 239 W.
Main St. Lancaster, Ohio.
What do I
need to submit with my application?
• A completed child care application. Be sure to read
and sign the Rights and Responsibilities page of the
application.
• Your most current 4 or 5 pay stubs or an employment
verification form completed by your employer.
• Current school schedule, if enrolled.
• Child support verification (paid or received).
• Family Household income statement
How long
will it take to approve my child care application?
The eligibility determination process will be completed
within 30 days from the date your application is submitted
to the office. An approval or denial will be sent to you.

When can my
child attend child care?
Child care services can be used only during times you
are actually at work, school or participating in an approved
OWF assignment. Use for any other reason (illness, holiday,
vacation of parent) is prohibited.
Why do I
have to sign my child in and out everyday?
Your daily signature verifies your child’s attendance
and authorized payment to the provider.
What is a
copay?
A monthly copay is determined for each family based on
household size and gross income. This copay is paid directly
to the provider by the parent.
How do I
become a child care provider?
Contact someone in the
child care unit to register.
