Transportation is available, to those who qualify for medical and counseling
appointments. The Non Emergency Transportation program is
designed to increase access to medical appointments. Gas vouchers
and direct transportation may be available for eligible
individuals.

Transportation
Who
is eligible?
1.
Customers residing in Fairfield County who are in current receipt
of Medicaid may qualify.
2. Customers attending a Medicaid-reimbursable medical
appointment.
3. Individuals who have met their Spenddown for the month of the
transportation request.
Services
available:
Gas
Vouchers - Individuals who have their own transportation or
have friends or family with a vehicle may receive gas vouchers. A
limit of eight vouchers per month may be available to an
assistance group.
Direct
Transportation - Those individuals who do not have access to a
vehicle may arrange transportation with one of the FCJFS driving
staff or outside contracted agencies. Curb-to-curb and door-to-door services may be available for
those who qualify.
How
to Apply:

All
requests for transportation must be made at least 5 business
days notice
prior to the appointment.

free 1-800-450-8845 to see if you qualify.

All transportation customers must sign a
Rights and Responsibilities
form which should be returned to the transportation
department to be kept on file.

Social Security numbers must be provided by the customer
requesting transportation. Other necessary information
includes the doctor's name and address, appointment dates
and time of appointment.

Transportation
Other
transportation may be available for those in receipt of public
assistance. This transportation is limited to specific activities.
